CRC's work in partnership with our Office of Population Health staff and community organizations to connect patients with resources to address their social needs, such as transportation, mental health, housing, food, financial, and social support. CRC's will: Review patient referrals, based on a completed social needs assessment, from the care team. Communicate with patients to identify and understand needs. Utilize a resource database to connect patients with local resources in transportation, housing, food, financial and social support. Initiate contact with the community-based organization, depending on the patient’s desired level of support. Initiate follow up with patients to ensure they connect with the resources needed. You can help third-graders in Tomah's Miller Elementary School master this key life skill, as a Read to Success tutor with The Great Rivers United Way! Studies show if a child is not up to grade level reading by the end of third grade, they almost never catch up. Read to Success runs throughout the majority of a school year, generally October - Mid May. Over the course of the 2025 - 2026 school year, you will work with one or two students, spending 30-45 minutes with them each week during the school day. No experience is required but volunteers must be able to walk and be on their feet for extended periods of time and be okay walking in sand. Volunteers provide direct support to our riders, many if whom have a diagnosis of some kind and often require physical support. Lessons are Monday-Thursday at 3pm, 4pm, 5pm, and 6pm each evening. We do ask volunteers to commit to the same day/time(s) for a full session to ensure consistency for our riders. Occasional absences are to be expected. The 2025 session dates are as follows: Spring: March 10th-May 16th Summer: June 2nd - August 22nd Fall: September 8th - November 17th Our needs have been met for the spring but we continue to look for support for the summer and fall.
